HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Your rights

You may inspect or obtain a copy of your health information; request corrections; ask us to communicate with you in a specific way; request restrictions; obtain a list of certain disclosures; receive a paper copy of this notice; and choose someone to act for you.

2. Your choices

For certain health information, you may tell us your preferences about sharing information with family, close friends, disaster-relief organizations, fundraising activities, and other circumstances permitted by law.

3. Treatment, payment and operations

We may use and disclose protected health information to treat you, coordinate care, bill and collect payment, operate and improve our practice, contact you about appointments, and perform other activities allowed by HIPAA.

4. Other permitted disclosures

We may disclose information when required by law; for public health and safety; health oversight; workers’ compensation; law enforcement; organ donation; medical examiners; certain government functions; and judicial or administrative proceedings, subject to applicable safeguards.

5. Specially protected information

Additional federal or Florida protections may apply to substance-use-disorder records, psychotherapy notes, HIV/AIDS information, genetic information and other sensitive records. We will obtain authorization when the law requires it.

6. Our duties

We are required to protect the privacy and security of protected health information, notify affected individuals following a breach when required, follow this notice while it is in effect, and provide an updated notice if our practices materially change.

7. Complaints and contact
